How to create a good customer survey?

Does anyone know if there is currently an effective means of collecting customer feedback?

What I want to do is survey our current shipping wine club members and find out who their preferred delivery system is. I have been getting a lot of complaints with who we use, but I don’t know if it’s a vocal minority or if most people prefer the “other carrier”. Basically, I want to create a survey like those built on Survey Monkey (seems like it should be easy considering the form builder already in Vin65). Has anyone done this with success? If so, how?

I’ve just used a Form, and emailed a link to it. The end result is a spreadsheet you can tally up in. I suggest making a test Form to understand the process. (And learn excel pivot tables if you need to.)

By sending out a survey however you do it, you’re still only sampling those who care enough to answer, and the happy people won’t necessarily care enough. Bribes can mitigate this, however.
